Monday & Wednesday: 8:00 am to 5:00 pm (closed for lunch 11:45 am to 1:00 pm) 
Friday: 8:00 am to 5:00 pm (closed for lunch 11:45 am to 1:00 pm)

The City of San Marcos City Clerk’s Office maintains all births or deaths records that occurred within the City of San Marcos. The City Clerk’s Vital Statistics Office receives, records and issues birth and death certificates, amendments to birth and death certificates, and burial transit permits.

This office also serves as a remote birth certificate printing location for the Texas Department of State Health Services. Certified birth certificates for any individual born in the State of Texas can be issued upon request.

Who can request a certified copy of a birth or death certificate?

For births within the past 75 years and deaths within the past 25 years, only the immediate family members of the person whose name is on the birth certificate or death certificate are eligible to request a copy.  Those with any other relationship must provide legal documentation, such as a court order establishing guardianship. If you are the legal representative of a qualified applicant, you must provide a release which documents a direct and tangible interest in the record you are requesting.
 

Proof of Eligibility

Each applicant must present a valid government-issued photo identification along with their application.

Mail-In Application

Mail the following items:

   • Completed and notarized application (download and print    
     application).
   • Check or money order payable to the City of San Marcos
       Note: Personal checks must be signed by the same person who    
               signs the application.

    Copy of your valid government-issued photo identification.
      See a list of accepted forms of ID.
   • Postage-paid (stamped) self-addressed envelope (we cannot mail birth
     certificate without the stamped envelope).
